Ontario Creates Self-Identification Questionnaire
What are we doing?
Under Ontario Creates’ most recent strategic plan the agency has committed to help to build the province’s creative workforce to better reflect the diversity of Ontarians. Ontario Creates is asking individuals in key business and creative roles associated with program and tax credit applications to voluntarily complete a confidential self-identification questionnaire that will help the agency measure progress towards this goal.
What is being collected?
Data is collected on a voluntary basis and covers Indigenous identity, racial and/or ethnic identity, gender identity and expression, belonging to the 2SLGBTQIA+, community, disability status, and belonging to an Official Language Minority Community. Ontario Creates has partnered with Telefilm to use their questionnaire, which was developed through extensive community consultation. More information on Telefilm Canada’s demographic data collection can be found on their website.
How will Ontario Creates use this data?
By filling the self-identification questionnaire, participants contribute to a culture that supports all creators, and the industry’s progress towards greater diversity goals.
Data collected may be used for:
• Improving and evolving Ontario Creates’ programs and services
• Identifying areas to support underrepresented communities
• Monitoring, evaluating, Ontario Creates’ performance
• The data will not be used to assess individual applications and will only be available to Ontario Creates in an aggregated format
How does it work?
When applying for program funding or a tax credit the applicant will list the project’s key personnel in both creative and business roles and will provide their contact information. Identified key personnel will receive an email from Ontario Creates inviting them to submit their self-identification questionnaire on a confidential basis. Recipients may also choose to decline. If no response is received, a reminder email will be sent approximately two weeks after the initial email. Once an applicant has completed a profile they will not be asked to do so again, even if they are associated with a subsequent application.
How do I update or delete the information I have provided?
Applicants are welcome to update their profile or request that it be deleted at any time by sending an email to research@ontariocreates.ca.
