Please help us to prepare for your consultation call by answering the questions below. The information that we are collecting will help us assess your eligibility for this program and be prepared to answer any questions that might come up during the call.

A few things to note:

1. This is not an application to the IDM Fund. The application form is available on the Ontario Creates Online Application Portal.

2. Your application will be reviewed for eligibility and completeness after the deadline. It is your responsibility to ensure that your application material clearly demonstrates your eligibility for the program. Information provided via this form and in a consultation call is not considered part of your application.

3. Consultation calls are offered as a courtesy to potential applicants. They are not a replacement for reviewing and understanding the guidelines.

Please provide the resume of an owner or full-time employee with your company who:

• has a minimum full-time equivalent of three years creating professional, commercialized content for (1) interactive platforms, networks or devices, (2) publishing industries (3) linear video content creation. OR
• has a producer credit on at least one linear video content project that has been made publicly available to an audience.
